• Please schedule your flu appointment by calling 512.245.2161.
• The consent form MUST be received before your appointment.

For All Other Immunizations

• Please have your parents review the applicable Vaccine Information Statement (VIS).

  • Hepatitis B
  • HPV (Gardasil)
  • Meningoccocal (Meningitis)
  • Measles, Mumps, Rubella (MMR)
  • Tetanus, Diphtheria (Td) or Tetanus, Diphtheria, Pertussis (Tdap)
