• All students enrolled at Texas State University are eligible and may participate in the plan.
• You are not required to be enrolled in a minimum number of hours to purchase the insurance.
• Students who are enrolled in the student health insurance plan during the spring semester are eligible to purchase insurance for the summer without being enrolled in classes during the summer months.

• It is recommended that all students have some form of medical insurance; however, only international students are required to have health insurance as a condition of enrollment.

• Please verify the accuracy of your Texas State student ID number prior to enrolling in the insurance plan. Providing an incorrect student ID number may result in the delay of verifying your insurance status and/or processing your claims.
• You must fill out an enrollment form in order to purchase the Texas State Health Insurance Plan. The domestic student enrollment form is for students and their dependents who are legal citizens and residents of United States. Premium and policy period information is available on this form.
• International students are automatically enrolled in the Texas State Health Insurance Plan when they register. If you are an international student and wish to add your dependents to the plan, you can download the international student dependent enrollment form here.

• Limited benefits are provided for mental and nervous disorders on an outpatient basis, and include dispensed prescription medication and lab testing conducted at the Student Health Center.

• No benefits will be paid for conditions included in the exclusions and limitations section of the brochure (see pages 10, 11 and 12) and for any pre-existing conditions. Please review this information carefully before purchasing the plan.

• Recent graduates who have been continuously insured under the school's regular student policy for at least three consecutive months and those who no longer meet the eligibility requirements under that policy have the option of purchasing continued coverage for themselves and their dependents. This coverage is offered on a monthly basis but may not exceed six months.
