• All students who are enrolled at Texas State University are eligible and may participate in the plan.
• You are not required to be enrolled in a minimum number of hours in order to purchase the insurance.
• Students who are enrolled in the Student Health insurance plan during the spring semester are eligible to purchase insurance for the summer without being enrolled in classes during the summer months.
• Domestic students can download an enrollment form here. This form includes the current premiums.

• It is recommended that all students have some form of medical insurance, however, only international students are required to have health insurance as a condition of enrollment.

• Please verify the accuracy of your Texas State student ID number prior to enrolling in the insurance plan. Providing an incorrect student ID number may result in the delay of verifying your insurance status and/or processing your claims.
• You must fill out an enrollment form in order to purchase the Texas State Student Insurance Plan. The domestic student enrollment form is for students and their dependents who live in the United States and are United States citizens. Premium and policy period information is available on this form.
• International students are automatically enrolled in the Texas State Student Insurance Plan when they register. If you are an international student and wish to add your dependents to the plan, you can enroll online.

• Recent graduates who have been continuously insured under the school's regular student policy for at least three consecutive months and who no longer meet the eligibility requirements under that policy have the option of purchasing continued coverage for themselves and their dependents. This coverage is offered on a monthly basis but may not exceed six months.
• To enroll in the plan, fax a completed Continuation Plan Enrollment Form to UnitedHealthcare StudentResources Premium Dept at 469.229.5612. For more information contact UHCSR Customer Service at 888.889.3618 or e-mail customerservice@uhcsr.com. Online enrollment is not available for the Continuation Plan.

• If the requirements for continued coverage through the school's policy are not met, short-term health insurance coverage is available through Student Resources for graduating students to consider. Follow this link for further information.

• The deductible will be waived.
• Benefits will be paid at 100 percent of covered medical expenses.
• No benefits will be paid for exclusions and limitations.
• Copays apply to prescriptions for students and non-registered students.

  • $20 for name brands
• Limited benefits are provided for mental and nervous disorders on an outpatient basis, as well as for prescription medication dispensed and lab testing conducted at the Student Health Center.
• The Student Health Center will bill the insurance company for covered medical expenses.

• No benefits will be paid for conditions included in the exclusions and limitations section of the brochure (see pages 17,18  and 19) and for any pre-existing conditions. Please review this information carefully before purchasing the plan.
